APPLICATION FOR FINANCIAL AID
2008-2009
Parents requesting financial aid are required to fill out this form. Please submit this form with a copy of your 2007 Federal Income Tax Form 1040A or 1040 with all schedules, and W-2 Forms. In divorced and separated families, both parents must submit this application form with their tax forms, either jointly or separately. Income from both households will be used in calculating financial aid. If you do not file a tax return, but are applying for financial aid, you must still complete this application. Parents whose sole source of income is public assistance should fill out the applicable information and submit income verification from their caseworker. All information reported is kept confidential. It is necessary this year to have a tax return with all Financial Aid Applications. If you won’t have one available by the due date, please fill out an estimated 1040 or 1040a tax return, sign it and return with your application. A final tax return is required by Feb 15, 2008.
